Efficacy of nimodipine combined with statins in the treatment of patient with subarachnoid hemorrhage and its impact on neurological function: a Meta-analysis

Abstract

Objective  To systematically review the efficacy of nimodipine combined with statins in the treatment of patients with subarachnoid hemorrhage (SAH) and its impact on neurological function.

Methods   PubMed, Embase, Cochrane Library, CNKI, SinoMed, VIP, and WanFang Data were searched electronically for randomized controlled trials (RCTs) comparing nimodipine combined with statins versus nimodipine alone in patients with SAH from inception to September 30, 2025. Two researchers independently screened the literature, extracted data, and assessed the risk of bias of the included studies. Meta-analysis was performed using RevMan 5.4 and R 4.4.2 software.

Results  A total of 17 RCTs involving 1,532 patients were included. The results of the Meta-analysis showed that nimodipine combined with statins significantly improved the clinical effectiveness rate compared with nimodipine alone [OR=3.51, 95%CI (2.55, 4.84), P<0.05]. Subgroup analysis by statin type revealed that both atorvastatin and simvastatin were beneficial, with no statistically significant difference between subgroups (P=0.505). The combined therapy reduced the degree of neurologic deficit [SMD=-1.92, 95%CI (-2.67, -1.16), P<0.001]. The incidence of adverse reactions was not statistically significant between the nimodipine combined with statins group and nimodipine alone group [OR=0.60, 95%CI (0.28, 1.29), P>0.05].

Conclusion  Nimodipine combined with statins is superior to nimodipine alone in preventing delayed cerebral ischemia and improving neurological function in SAH patients, and it has good safety. Due to the limited number and quality of included studies, the above conclusions still need to be verified by large-sample, multicenter and high-quality RCTs.
