Objective To systematically evaluate the efficacy and safety of platelet-rich plasma (PRP) for androgenetic alopecia (AGA).
Methods PubMed, Embase, Web of Science, the Cochrane Library, CNKI, Wanfang Data, and VIP were searched from inception to March 2026 for randomized controlled trials (RCTs) of PRP-related therapy for AGA. Two reviewers independently performed study selection, data extraction, and risk-of-bias assessment. Meta-analysis was conducted using RevMan 5.4 and R software.
Results A total of 16 RCTs were included. Compared with control treatment, PRP-related therapy significantly improved the overall clinical response rate [RR=1.25, 95%CI (1.11, 1.40), P<0.001], hair density[SMD=1.52,95%CI(0.93, 2.11),P<0.001], follicular density [SMD=1.53, 95%CI (1.07, 1.99), P<0.001], and hair diameter [SMD=1.37, 95%CI(0.66, 2.07), P<0.001] in patients with AGA. Reported adverse events were mainly mild and self-limiting local reactions, including transient pain, erythema, pruritus, punctate bleeding, edema, and scalp tightness; no serious adverse events were reported.
Conclusion Current RCT evidence suggests that PRP-related therapy can improve the overall clinical response rate, hair density, follicular density, and hair diameter in patients with AGA, with generally favorable short-term tolerability. Due to the limited quality and quantity of the included studies, more high-quality studies are needed to verify the above conclusion.
